Crit (crit.org) does something similar to this via creating 'external links', links that are not in the original document but in a separate database.

Perhaps a more immediate solution is for everyone who publishes lengthy technical documents to use 'a name' tags for each paragraph or table - even if never referenced in the document, it would allow others to make such in-document links.
